PHOTO RELEASE By registering for this event you give FBC Rowlett permission to use your child’s likeness in publications and posts. The undersigned authorizes give FBC permission to use and display any said photographs in publications, multimedia productions, displays, advertisements or internet publication related to the promotion of FBC or in the promotion of any activities supported by FBC. RELEASE I willingly allow my son/daughter to participate in events/programs organized by First Baptist Church Rowlett, Texas. In the event that he/she is injured and requires the attention of a doctor, I consent to any reasonable medical treatment as deemed necessary by a licensed physician. In the event treatment is required from a physician and/or hospital personnel designated by the Church, I agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of such consent. I also acknowledge that I will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by health insurance provider. Further, I affirm that the health insurance information provided above is accurate and active for any care my child may require.

If your child is attending an off-campus event with First Baptist Rowlett and he or she is under the age of 8 and under 4’9", you, the parent, are required to supply a federally approved safety seat for your child, You must install your child’s car seat in the vehicle.

Each child must have a completed Ninja Nation safety waiver to attend Spring Fling. By registering your child for Spring Fling you agree to complete the safety waiver before the start of the event. A link to complete the safety waiver will be emailed to you after you register your child here.
